Position Summary

We are seeking a compassionate, detail-oriented Universal Care Assessor to join our team within the Community First Choice (CFC) Program. In this vital role, you will assess individual needs, develop person-centered care plans, and coordinate community-based services that promote independence and well-being. Your work will make a meaningful difference in the lives of those we serve throughout South Central Connecticut.

Key Responsibilities
  • Conduct comprehensive, in-home assessments to determine participant needs, strengths, and preferences.

  • Develop and implement individualized care plans in partnership with participants, families, caregivers, and service providers.

  • Monitor ongoing service delivery to ensure care plans remain aligned with client goals.

  • Serve as an advocate for participants by addressing concerns and supporting access to appropriate resources.

  • Maintain accurate, timely documentation of assessments, plans, services, and client communications in electronic health records.

  • Build and maintain effective relationships with community partners to enhance service delivery networks.

  • Participate in continuing education and professional development to remain current with best practices and program requirements.

Qualifications
  • Bachelor’s degree in Social Work, Nursing, Human Services, or a related field required.

  • Minimum of two (2) years’ experience in care coordination, case management, or a comparable role.

  • Strong knowledge of the aging and disability service systems, with a client-centered approach.

  • Excellent communication, interpersonal, and organizational skills.

  • Ability to work independently while collaborating within a multidisciplinary team.

  • Proficiency in electronic documentation systems and Microsoft Office Suite.

Schedule
  • Full-Time | Monday – Friday | 8:30 AM – 4:30 PM
