Position Summary

AOASCC is seeking a
Care Manager Assistant to join our Home Care Program team.

The Care Manager Assistant provides client outreach, administrative support, and care coordination assistance to Care Managers serving older adults and individuals with disabilities who wish to remain safely and independently in their homes.

This position is ideal for someone who is organized, compassionate, detail-oriented, and comfortable managing a high volume of client outreach and follow-up.

Key Responsibilities
  • Complete a minimum of
    400 required monthly and quarterly client reviews each month by telephone or home visit, ensuring all contacts and outcomes are accurately documented in the client record in accordance with established documentation requirements.
  • When assigned responsibility for client satisfaction surveys, complete up to
    350 client calls per month
    .
  • Complete required follow-up resulting from client reviews, including confirming hospital or skilled nursing facility (SNF) admission and discharge dates, making appropriate referrals, and assisting with monitoring and adjusting client care plans with Care Manager approval.
  • Maintain at least monthly contact with pending clients and assist them in navigating the funding-source application process for the Connecticut Home Care Program for Elders (CHCPE).
  • Monitor and manage the
    DSS Memo Queue daily, identify the assigned Care Manager, and ensure appropriate notification and task delegation through the NOMMA client management system.
  • Provide administrative and clerical support to Care Managers and program coordinators.
  • Work collaboratively as a member of the Home Care team and provide backup support to other Care Manager Assistants, including coverage of the DSS Memo Queue.
  • Perform other related duties as assigned.
Qualifications
  • Two to five years of related experience and/or training, or an equivalent combination of education and experience.
  • Experience in human services, social services, healthcare, aging services, care coordination, or a related setting preferred.
  • Strong communication, organizational, and time-management skills.
  • Ability to effectively manage a high volume of client contacts, follow-up responsibilities, and competing priorities.
  • Strong attention to detail and ability to maintain timely and accurate documentation.
  • Compassionate and professional approach when working with older adults, individuals with disabilities, caregivers, and families.
  • Proficiency with Microsoft Office, including Outlook and Word.
  • Ability to learn and effectively use electronic client-management systems.
