Hope Health is seeking an experienced Registered Nurse (RN) for its Access Center in Providence, RI. The role serves as a liaison among referral sources, care providers, staff, patients and families to ensure smooth admission of patients to hospice services.
The Access Center RN is a full-time, 40-hour-per-week position with varied start times (8-4:30, 10-6:30) and a rotating third-weekend schedule. Responsibilities include scheduling, follow-up, and admissions support as needed to facilitate
