The Primary Care LPN provides complex age-specific nursing care for adult and geriatric patients with physical, mental, behavioral, or psychosocial needs, according to assignment and training in the outpatient clinic setting.

- The employee in this position has at least one-year qualifying experience as an LPN or equivalent and is expected to operate in accordance with all established policies and procedures. LPNs at this grade level perform all duties expected of LPNs at the level, however, they are expected to be technically proficient in initiating, performing, and completing assigned duties in providing care to variable patient populations, requiring minimal guidance and direction.

Demonstrated ability to observe, identify, and respond to the patient's needs for nursing, medication, and equipment-assisted care, and to recognize deviations from normal. In modifying care plans, the LPN considers physical, emotional, and social factors.

Practical knowledge of human behavior, motivations, and reactions to situations. Demonstrated ability to use this knowledge in working effectively with patients, families, and other staff members.

Patient care activities, as appropriate to assignment, (includes, but is not limited to):

* Provides for the patient's privacy and safety.

* Observes, reports and documents sign signs and symptoms of patient's general condition.

* Completes and documents clinical reminders as indicated.

* Collecting and labeling specimens, e.g., blood, sputum, urine, stool.

* Providing bowel/bladder training, applying external urinary devices, performing bladder catheterization.

* Setting up and initiating oxygen therapy, including nebulization treatments.

* Providing wound care and reporting any changes and/or progress to the RN and provider.

* Providing for lifting, moving, transferring, and/or transporting patients without any personal limitations or physical restrictions.

* Using patient lifting devices and electronic scales without any personal limitations or physical restrictions.

* Applying abdominal binders, anti-embolic stockings, ace bandages as ordered.

Work Schedule:

7:30 am - 4:00 pm with rotating shifts, subject to change based on the needs of the facility
