position summary

the clinical coordinator
is responsible for coordinating the interdisciplinary plan of care for assigned patients/families, providing appropriate interventions and support for the patients/families/caregivers by identifying their physical, psychological, social, economic and spiritual needs, and actively participating as a member of the interdisciplinary team by providing these interventions. The (apc) clinical coordinator will serve an integral role in the success of arkansas palliative care by providing high quality care to our patients and ensuring excellent customer service to our referring clinicians and partner facilities.

qualifications

education: high school diploma or ged; satisfactory completion of a nursing assistant (cna) training, licensed practical nurse (lpn), or medical assistant program (ma).

experience: previous experience in direct patient care in a hospital, long-term care facility, physician office, or hospice preferred.

licensure: certified nursing assistant (cna), licesenced practical nurse (lpn), medical assistant (ma)

additional requirements: must be eligible for a letter of determination and successfully complete a skills evaluation test. Valid arkansas driver’s license, car insurance, and car also required (home care). Ability to work flexible hours and occasional overtime required. Cpr certification required.

primary responsibilities

  • provides safe, high quality, and comprehensive coordination of care to assigned patients, families and caregivers; coordinates development and revision of the patient’s plan of care.
  • performs ongoing physical and emotional evaluation of patient/family; review patient/family/caregiver for response and acceptance; provides accurate information regarding services and expectations for care.
  • performs ongoing evaluations of the patient/family needs and coordinates the appropriate services (dme, home health, hospice) as required throughout patient’s transition.
  • in collaboration with the interdisciplinary team, provides communication and coordination at all levels from referral to throughout care transitions which may accompany serious illness.
  • obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status; incorporates the data into the plan of care, reports and program metrics as indicated.
  • promotes effective written and verbal communication with patients, families, interdisciplinary team members, and other health care providers. Initiates communication with the provider(s) and other community resources to report changes and coordinate optimal care and use of resources for assigned patients/families.
  • maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.
  • serve as a liaison between partner facilities and arkansas palliative care.
  • documents care given daily according to organizational guidelines.
  • recognizes and immediately reports to provider(s) any changes in the patient’s mental or physical condition, family situation, and/or emergencies.
