PositionSummary
This Bilingual Analyst, Case Management position is with Aetna's Long-Term Services and Supports (LTSS) team and is a field-based position out of North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL. Candidates must travel 50-75% of the time to meet with members face to face.
Schedule is Monday-Friday, 8:00am-5:00pm, standard business hours.
We are seeking self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our Case Management team. This opportunity offers a competitive salary and full benefits. The Case Management Coordinator is responsible for telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case management activities with members to evaluate medical needs and facilitate overall wellness.
The Coordinator will effectively manage a caseload that includes supportive and medically complex members, developing a proactive course of action to address issues and enhance short‑ and long‑term outcomes and overall wellness through integration. The Coordinator will determine appropriate services and supports due to members’ health needs, including but not limited to: prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources and supports.
- Bi‑lingual in English and Spanish.
- 1+ year of experience in behavioral health or a long‑term care setting.
- Computer proficiency in Microsoft Word, Excel, and Outlook.
- Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler FL.
- Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face‑to‑face in North Miami FL. Mileage is reimbursed per company expense reimbursement policy.
- Ability to multitask, prioritize and effectively adapt to a fast‑paced changing environment.
- Effective communication skills, both verbal and written.
- Managed Care experience.
- Case Management and Discharge Planning experience.
- Bachelor's degree required, preferably in Social Work or related field.
- Master's degree preferred.

Pay RangeThe typical pay range for this role is:
$21.10 - $40.90
The pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
