The MDS Coordinator plays a vital role in the intake, assessment, and care planning process for residents in our long‑term care community. Position Summary

The MDS RN Coordinator serves as a liaison throughout the resident intake and evaluation process and is responsible for maintaining accurate, timely resident assessments. This role coordinates and oversees the interdisciplinary assessment process for all nursing home residents, ensuring compliance with regulatory requirements and supporting optimal resident outcomes.

Key Responsibilities
  • Coordinate and complete MDS assessments in accordance with federal and state regulations.
  • Oversee the interdisciplinary assessment and care planning process.
  • Collaborate with nursing, therapy, social services, dietary, and other departments to ensure comprehensive resident care.
  • Monitor assessment schedules and ensure timely completion of required documentation.
  • Assist with resident admissions, evaluations, and care transitions.
  • Support quality improvement initiatives and regulatory compliance efforts.
  • Serve as a resource for team members regarding assessment and reimbursement processes.
Qualifications
  • Current Registered Nurse (RN) license.
  • Current CPR Certification.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and collaboration abilities.
  • High personal integrity and professionalism.
  • Compassionate, resident‑centered approach to care.
  • Positive attitude and commitment to quality outcomes.
