Gentiva Hospice is seeking a Hospice RN / RN Case Manager to lead clinical care in patients' homes and other settings. You will perform comprehensive assessments, coordinate with an interdisciplinary team, deliver skilled nursing interventions, and educate patients and families on end-of-life care.
You will develop individualized plans of care, monitor outcomes, ensure accurate documentation, and participate in on-call coverage with a focus on compassionate, patient-centered service.
