Advanced Practice Provider – Primary Care About the Role

At Thundermist Health Center, you'll serve as an autonomous licensed provider responsible for your own patient panel — building continuity, managing chronic and acute conditions, and delivering the kind of care that makes a real difference over time. - Manage a panel of primary care patients, providing continuity of care through scheduled appointments and ongoing management of acute and chronic conditions.
- Conduct comprehensive patient assessments including health histories, physical exams, and anticipatory guidance across the lifespan.
- Develop, implement, and update individualized care plans based on clinical assessment, patient goals, and evidence-based standards of care.
- Perform point-of-care testing including rapid strep, flu, COVID, urinalysis, STI testing, and more.
- Perform wound care, suturing, splinting, and minor surgical procedures.
- Order and interpret diagnostic tests (X-ray, ultrasound, labs) and initiate appropriate treatment based on results.
- Prescribe medications within your scope of practice.
- Maintain complete and accurate same-day clinical documentation.
- Supervise nurse practitioner students as assigned.
- Conduct peer chart reviews on a rotating basis.
- Participate in departmental and team meetings to support care coordination and quality improvement.
- Maintain compliance with credentialing, re-credentialing, and privileging requirements.
- Meet patient throughput and productivity standards to support efficient, accessible care.
You're a confident, thoughtful clinician who brings both clinical expertise and genuine warmth to every patient relationship. You're comfortable owning a panel, managing complexity, and motivated by the mission of making high-quality primary care accessible to everyone.
- 3+ years of experience as an NP or PA in a primary care setting, with demonstrated experience managing a patient panel.
- Active Rhode Island RN and NP or PA licensure.
- ANA Certification in your specialty (family medicine, internal medicine, women's health, or pediatrics).
- Ongoing continuing education to maintain and update clinical skills.
- Exceptional oral and written communication skills, with proficiency in applicable medical terminology.
- Demonstrated commitment to patient-centered, high-quality care for diverse and underserved populations, and an authentic passion for health equity and community health.
This is a full-time (40 hours/week) onsite position. 80% of your time will be patient-facing; 20% is dedicated to administrative duties, which can be completed from home or onsite. Clinic hours are Monday–Thursday, 8:00am–8:00pm, and Friday, 8:00am–5:00pm. At least one clinical day per week will include hours after 5:00pm to support patient access.
