Registered Nurse; RN - Home Health Admission - per visit

Position Summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health Admissions in North Reading, Massachusetts. The role is Full Time and focuses on completing OASIS Start of Care (SOC) visits for adult and geriatric patients across Northeast Massachusetts.

• Perform head‑to‑toe assessments and identify subtle changes in patient condition.
• Use acute and post‑acute care skills (CHF, COPD, wound care, IV therapy, post‑surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Conduct medication reconciliation, teach patients and families, and monitor safety.
• Perform comprehensive wound assessment, care, and documentation.
• Manage a dynamic visit schedule, prioritize urgent visits, and maintain productivity expectations.
• Ensure efficient point‑of‑care documentation and technology use.
• Adapt to changing patient needs and varying home environments.
• Provide culturally sensitive care that respects patient autonomy.

• Current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited nursing program, verified by transcript or diploma.
• At least one year of recent hands‑on medical experience with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab units.
• Experience with Medicare Certified Home Health and OASIS documentation.
• Ability to work independently while knowing when to seek escalation.
• Strong problem‑solving skills in unpredictable home environments.
• Excellent communication and time‑management abilities.
