The role will work across all locations including Bay Village, Fairview Park, Lakewood, Middleburg Heights, North Olmsted, and North Ridgeville to coordinate the MDS process and develop residents’ plans of care.
The ideal candidate has 2 years of MDS experience and an Ohio RN license.
