Home Health Registered Nurse (RN) | Wound Care | North Royalton | South Cuyahoga

Bright Star Care is seeking an experienced Registered Nurse (RN) with wound care experience to join our growing clinical team.

This role is ideal for an RN who is passionate about wound management, skilled nursing, and providing high-quality care in the home. You'll perform wound assessments, develop treatment plans, provide skilled nursing visits, and collaborate with physicians and the care team to promote optimal healing and patient outcomes.

Schedule / Location

Location: North Royalton

Schedule:

  • 7 days per week
  • 2-hour visits (AM, PM, or Evening)
  • Premium pay available
  • Must be available to travel throughout the West Cleveland service area
What You'll Do Wound Care
  • Perform comprehensive wound assessments and reassessments
  • Provide wound dressing changes and treatments as ordered
  • Monitor wound healing and identify signs of infection or complications
  • Develop and update individualized wound care plans

    Measure, document, and photograph wounds as appropriate
  • Educate patients and caregivers on wound care, infection prevention, and skin integrity
  • Collaborate with physicians regarding treatment recommendations and progress
Skilled Nursing Care
  • Complete patient assessments and nursing evaluations
  • Administer medications, injections, and other prescribed treatments
  • Perform catheter care and other skilled nursing procedures
  • Monitor patient conditions and report changes promptly
  • Maintain accurate and timely clinical documentation
  • Follow infection prevention and safety protocols
Case Management & Care Coordination
  • Develop and maintain individualized Plans of Care
  • Coordinate care with physicians, families, therapists, and interdisciplinary teams
  • Support new client admissions and continuity of care
  • Ensure compliance with Bright Star Care policies, Joint Commission standards, and state and federal regulations
Clinical Support
  • Conduct supervisory visits as assigned
  • Provide education and support to caregivers and nursing staff
  • Assist with orientation and competency validation
  • Participate in quality assurance and chart review activities
Qualifications Required
  • Current unrestricted Ohio Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum 3 years of clinical nursing experience
  • Experience providing wound care
  • Current CPR certification
  • Valid driver's license
  • Reliable transportation with current auto insurance
  • Ability to travel throughout assigned service areas
  • Ability to pass a background check, drug screen, and other pre-employment requirements
Preferred
  • Home Health experience
  • Wound Care Experienced is required
  • Experience with chronic wounds, pressure injuries, diabetic ulcers, and surgical wounds
Physical Requirements
  • Ability to stand, walk, bend, kneel, reach, push, and pull throughout the workday
  • Ability to safely lift up to 50 pounds with or without reasonable accommodation
