Case Manager - Long Term Care and Developmental Disabilities Waivers

HOURS AVAILABLE

Monday - Friday 8:00am - 4:30pm, with some flexibility. Starting wage $52,000+ based on qualifications and experience.

Business office is located in North St. Paul, MN.

Full-Time Case Manager needed to assess needs, create/review care plans and coordinate services with individuals who qualify for CADI, DD and/or BI waivers. Waiver case managers make referrals for individuals served to community resources based on their assessed need, monitor ongoing need for changes in support services for the individual and ensure waiver coordination/documents meet all necessary deadlines and meet lead agency guidelines.

• 4 year degree in Human Services Field (Social Work, Psychology, Sociology) or closely related field.
• 1 year experience working with individuals diagnosed with Mental Illness and/or Developmental Disability.
• Driver's license, reliable vehicle and proof of insurance.
• Ability to travel throughout the Twin Cities / Metro Area is required. Ability to meet face to face with individuals in their homes and community. Based on the caseload, there may be travel outside the Metro Area.
• Must pass background check.
• Good verbal/written communication, organized, flexible and motivated.
• Strong time management and multitasking skills are essential to this position.
• Experience working with CADI, BI and/or DD waivers preferred.

Full-time Case Management

From $52,000.00+ per year based on qualifications and experience in the social services field working in positions such as IHS, ARMHS, Mental Illness and/or Developmental Disability residential facilities, employment programs etc.
