- What You'll Do

As a Home Health RN, you'll be the heart of patient care-bringing clinical excellence directly into patients' homes.

* Manage a dynamic caseload of patients in your local community

* Perform comprehensive assessments and develop individualized care plans

* Provide hands-on skilled nursing care and patient education

* Monitor patient progress and adjust care plans as needed

* Collaborate with physicians, therapists, and interdisciplinary teams

* Be a trusted partner for patients and families throughout their care journey

- What You'll Need

* Active, unrestricted RN license

* At least 1 year of clinical nursing experience

* Ability to travel locally (you'll spend time in the field-this is not a desk job)

* Valid driver's license, reliable vehicle, and current auto insurance

* Ability to safely assist patients (lifting, positioning, etc.)

* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.

Preferred (but not required):

* Home Health or Hospice experience

* Familiarity with Home Care Home Base

* Associate's or Bachelor's Degree in Nursing
