We are currently seeking a Full-Time Home Health Registered Nurse (RN) Case Manager to join our growing team serving the Northampton and surrounding communities. What You'll Do

As a Home Health RN Case Manager, you will manage a caseload of patients and provide skilled nursing care in the home environment while developing your clinical expertise in a growing and dynamic setting.

* Perform comprehensive patient assessments and develop individualized care plans

* Conduct initial and ongoing skilled nursing visits based on patient needs and physician orders

* Monitor and evaluate patient status, including physical, psychosocial, environmental, and caregiver factors

* Coordinate care with physicians, therapists, social workers, and other care team members

* Communicate changes in patient condition and recommend appropriate interventions

* Update and revise care plans as patient needs evolve

* Provide education and support to patients and caregivers

* Maintain accurate, timely clinical documentation in compliance with regulations and company standards

* Ensure adherence to HIPAA and patient confidentiality requirements

What We're Looking For

* Current, unrestricted Registered Nurse (RN) license in the applicable state

* Minimum of 1 year of nursing experience in a clinical setting

* Valid driver's license, auto insurance, and reliable transportation

* Willingness to travel within the assigned territory (approximately 50%);

* Ability to sit, stand, bend, lift, and move intermittently

* Ability to lift 50-100 lbs as needed
