Licensed Practical Nurse

We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide in‑home healthcare services. The ideal candidate will have strong clinical skills, attention to detail, and the ability to deliver high‑quality care in a home health setting. This role requires experience with Epic Homecare, strong communication abilities, and a proactive approach to patient care.

• Evaluate and monitor patients’ progress in the home setting
• Identify and respond to sudden or subtle changes in a patient’s medical condition
• Communicate updates clearly to physicians, patients, and family members
• Begin treatment and monitor medication dosages
• Respond to medical emergencies and notify appropriate healthcare providers
• Complete all necessary documentation prior to patient transfers
• Maintain up‑to‑date and accurate patient records
• Develop and implement effective, individualized care plans

• Education:
  
  Bachelor of Science in Nursing (BSN)
• Experience:
  
  Minimum 2 years of current LPN experience, specifically in Med Surg unit
• Technical
  
  Skills:
  
  Experience with Epic Homecare system
• Licensure:
  Active LPN License

• Basic Life Support (BLS) – Required

• Strong interpersonal and communication skills
• Ability to work independently in a home‑based care environment
• Experience handling medical emergencies in a non‑hospital setting
• Organizational skills and attention to documentation accuracy
