Licensed Practical Nurse (LPN) – Home Health

Contract Duration: 13 weeks + probable extension

Shift: Day

Pay Rate: $37/hr - $40/hr

• Provide skilled nursing care to patients in their homes under the direction of the RN Case Manager and Clinical Team Manager.
• Perform patient assessments and nursing interventions within the scope of the LPN license.
• Deliver care in accordance with the State Nurse Practice Act, organizational policies, and federal, state, and local regulations.
• Administer medications and treatments as prescribed.
• Monitor patient conditions and report changes to the RN Case Manager.
• Accurately document patient care, treatments, and progress in the electronic medical record (EMR).
• Maintain infection control and patient safety standards.
• Complete an average of 6 home visits per day

• Minimum 1 year of Home Health LPN experience required.
• Experience providing skilled nursing care in a home health setting.
• Knowledge of home health regulations and nursing standards.
• Strong clinical assessment and documentation skills.
• Ability to work independently and manage multiple patient visits daily.

• Graduate of an accredited Practical Nursing (LPN) program.

• Current Licensed Practical Nurse (LPN) license.
• Current Basic Life Support (BLS) certification.
