About This Role

The Case Manager RN is responsible for providing direction and leadership for patient progression and care coordination, collaborating with various disciplines to ensure effective patient care from admission to discharge.

• Pay: $43.38 - $74.94/Hourly

• Schedule:
  
  Per Diem, scheduled as needed

• Direct and coordinate patient care from admission to transfer or discharge.
• Collaborate with internal and external disciplines for effective care coordination.
• Advocate for patients to minimize out-of-pocket expenses.
• Ensure cost-effective utilization of hospital resources.
• Operate within established organizational and departmental policies and procedures.

• Education: Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred
• Experience: 1-2 years of related clinical, continuing care, or case management experience preferred
