-4:30 pm with weekend rotation of 1 /4 weekend requirement. The LPN position provides 6 patient visits per day and works within teams of RNs, PT, OTs, HHAs. This position requires home care experience. Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for performing skilled nursing functions in accordance with the State Nurse Practice Act and acceptable standards of practice, federal, state, and local laws and within the guidelines of his/her professional license.
The LPN delivers care to patients as delegated by the RN Case Manager and/or Clinical Team Manager. This position is required to work one weekend shift per month.
