MDS Coordinator

Alliance Health at Coleman is recruiting for an RN Case Manager 16-24 hours per week.

• Assess and evaluate the quality of care being given to nursing facility residents per CMS guidelines.
• Ensure completion of MDS interviews and assessments following the RAI process.
• Develop a plan of care that has measurable goals collaborating with residents’/caregivers.
• Ensuring regulatory compliance of Medicare, Medicaid, and Managed Care by following federal and state guidelines.
• Coordinate with multidisciplinary team to ensure implementation of the residents’ plan of care, assessments, and education.
• Conduct and attend meetings with the multidisciplinary team, caretakers, and families.
• Current/active Massachusetts Registered Nurse license.
• Must have 3-5 years' experience as an MDS Nurse in a Long-Term Care Facility.
• RAC-CT certification preferred.
• BLS CPR certification.
