The LPN II provides direct service and daily nursing care in a medically intensive program designed to meet the needs of a unique and specialized population diagnosed with a brain injury, developmental delay, or intellectual disability and with complex medical needs. The LPN II assumes overall responsibility for the program during the designated shift as lead staff/nurse and ensures that care is delivered in a respectful, dignified, high‑quality manner, promoting individual choice.
NursingCare
- Implements person‑centered treatment care plans in accordance with medical orders.
- Assists with activities of daily living (ADLs) per individual plan, including showering, dressing, shaving, mouth care, foot care, and diabetes care.
- Performs all skilled nursing care needs, including wound care, GT/JT care, trach care, and nutritional support.
- Monitors and observes clients for changes in status and reports all issues promptly to the Program Director, Program Supervisor (if needed), and Program Nurse.
- Ensures care is delivered using best practices while respecting client dignity and choice.
- Maintains ADLs and ensures assistive devices are in working order and ready for use.
- Conducts individualized assessments with any status change and provides indicated skilled nursing interventions.
- Clinically supervises CNAs during shifts, providing in‑moment training when needed.
- Ensures care is completed accurately and in a timely manner.
- Ensures care meets all ISP goals and addresses the whole person, not just medical needs, and that program quality control standards are met.
- Communicates with the RN Manager as required.
- Maintains communication with primary care providers, family members or guardians, and other significant stakeholders.
- Collaborates effectively with Vinfen employees, outside agencies, community agencies, and neighbors.
- Documents nursing care and treatments as ordered by the primary care provider in accordance with Vinfen policies and procedures.
- Records medication administration per Vinfen policy, following MAP standards.
- Ensures all documentation is complete and accurate, including shift notes, daily communication, treatment protocol data, medication administration sign‑offs, and the countable controlled substance book for each individual.
- Reviews CNA and direct care staff documentation for completeness.
- Ensures the environment is safe, clean, comfortable, and welcoming to clients, families, and visitors.
- Verifies environmental compliance with all regulatory requirements.
- Promotes a setting that encourages client participation in residency.
- Respects client preferences and choices.
- Experience working with individuals who have developmental, intellectual disabilities, or brain injury preferred.
- Strong commitment to the rights and abilities of individuals with complex health issues.
- Knowledge of LPN practice standards and intervention techniques.
- Sensitivity to cultural, religious, ethnic, age, gender, and disability issues.
- Capacity to develop supportive, trusting relationships with clients.
- Knowledge of teaching and learning modalities.
- Ability to supervise and mentor CNAs.
- Familiarity with evidence‑based practices and commitment to adherence.
- Proficiency in varied communication and learning styles and supervisory techniques.
- Organizational, strategic, participatory, and collaborative skills.
- Understanding of formal and informal assessment practices.
- Valid LPN license in Massachusetts.
- High school diploma or equivalent required; associate’s degree preferred.
- One to two years of supervised practical experience, including internships, in a clinical or rehabilitation setting preferred but not required.
- Experience with developmental and/or psychiatric disabilities and co‑occurring disorders preferred.
- Preference for bilingual/bicultural applicants and those with lived experience.
- Strong commitment to the rights and abilities of individuals with complex health issues.
- Sensitivity to cultural, religious, ethnic, age, gender, and disability issues.
- Must become CPR‑certified within two weeks of hire; training provided by Vinfen.
