Alliance Health at Coleman is looking for an RN Case Manager to work 16-24 hours per week. The candidate will assess and evaluate the quality of care for nursing facility residents, develop care plans, and ensure compliance with regulatory guidelines.
This position requires a current Massachusetts RN license, 3-5 years of MDS Nurse experience in a Long-Term Care Facility, and BLS CPR certification is preferred. Collaboration with a multidisciplinary team is essential.
