Position Overview
Licensed Practical Nurse in Home Health. Provide quality patient care and perform technical skilled care in patients' homes.
Primary Responsibilities- Provides high quality clinical services within the scope of practice, as defined by state laws governing nursing, in accordance with the plan of care, and in coordination with other health-care team members, following all applicable infection control procedures.
- Documents patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record using the agency's electronic visit verification program.
- Actively participates in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers (as applicable).
- Current and unrestricted LPN licensure in state of practice.
- Current CPR certification.
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
- Ability to work flexible hours to meet identified client needs.
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
- Home care experience.
- Ability to manage multiple tasks simultaneously.
- Able to work independently.
- Good communication, writing, and organizational skills.
Pay Range: $56,911 - $85,367 annual total cash target pay; $27.36 - $41.04 per visit point; $32.83 - $49.25 hourly rate. Annual total cash compensation assumes full-time employment (40 weekly hours). Compensation includes earnings from per-visit point pay and hourly pay, based on factors such as local labor market, education, and work experience. Two types of compensation are awarded: per-visit pay and hourly pay for non-visit activities.
