Residential Home Health and Hospice ("Residential") is looking for a compassionate home health registered nurse case manager to lead and collaborate with a team to provide management and delivery of patient care plans for patients in Northbrook and surrounding areas.
Key Responsibilities- Lead and coordinate care plans for patients, ensuring timely and accurate charting.
- Travel approximately 20% of the day to patient locations, maintaining a flexible schedule.
- Collaborate with clinical educators, preceptors, supervisors, and the after‑hours care team.
- Utilize electronic medical records, tablets, and mobile phones to document patient encounters.

- RN license in Illinois.
- Graduate of an approved practical nursing program.
- Minimum one year of RN experience with clinical competence.
- Current driver’s license and ability to travel to patient sites.

Compensation includes patient care visits, non‑visit activities, paid time off, shift premium pay, and a first‑year success bonus. Low end of the range is the average annual compensation for a fully productive caseload; high end reflects those who exceed productivity. Compensation potential varies by market.
