We are seeking a compassionate and experienced Registered Nurse (Home Health) to join our client’s dedicated team in Northbrook, Illinois. This position focuses on providing high-quality, individualized nursing care to patients in their homes, supporting their comfort, recovery, and independence. The ideal candidate demonstrates excellent clinical skills, strong communication, and a passion for patient-centered care.

• Perform skilled nursing assessments and deliver personalized care to patients in home settings
• Develop, implement, and update individualized care plans in coordination with physicians and care teams
• Educate patients and families on medications, treatments, and disease management
• Document care accurately and in compliance with clinical and regulatory standards
• Communicate effectively with interdisciplinary team members to ensure continuity of care

• Monday through Friday, day shift
• 1 weekend per month, 2–3 on-call days per month, and 2 holidays per year
• This is a Pay Per Visit position

• Local travel within the Northbrook area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (Home Health experience preferred)
• Current BLS or ACLS certification through the American Heart Association
• Valid driver’s license, reliable transportation, and active auto insurance

• Strong clinical and time management skills
• Excellent interpersonal communication and patient education abilities
