Job Description

Job Title: Home School Coordinator

  • Coordinates the Act 264 process including facilitation of interagency meetings and preparation of coordinated services plans.
  • Provides home visits to help families increase access to services and resources, address student attendance problems.
  • Assist with developing attendance plans for students as needed.
  • Serve as a liaison between home and school when considerable support is necessary, as in welfare cases, foster home children, and disadvantaged students.
  • Develop awareness of school policies, procedures and resources. Accesses proper personnel to answer parents’ and guardians’ questions and resolve conflicts as needed.
  • Respond appropriately and cooperate with parent/guardians, students, and staff to resolve matters in the best interest of students.
  • Provides assistance to school when a student is in a mental health crisis.
  • Provides staff training and assistance with child abuse and neglect reporting.
  • Collaborates with all school teams, as appropriate.
  • Provides consultation to staff, administration, and families regarding social/emotional issues.
  • Research and pursue grant opportunities relevant to services for students.
  • Maintain current knowledge base and skills to provide and coordinate home school supports and increase skills through relevant professional development.
  • Provides confidential mental health referrals.
  • Maintains data and professional records regarding students and services.
  • Coordinates Wrap Around Services for intensive social/emotional (non‑IEP).
  • Facilitates coordinated service plans and meetings.
  • Organizes small group adventure based programming.
  • Serves as a resource for DCF reporting.
  • Manages the data about children supported by DCF and homeless services or currently working with law enforcement.
  • Serves as truancy officer.
  • Serves as a member of the Targeted/Intensive Social/Emotional Support team.
  • Co‑responds to students in crisis.
  • Consults at the Universal level around social/emotional supports.
  • Supports and coordinates family engagement strategies (i.e. parent nights, learning fairs, etc.).
  • Builds connections to outside agencies.
  • Transport parents to school meetings.
  • Help parents navigate the education system.
  • Supports the efforts of local law enforcement.
  • Coordinates parent supports and tools including referrals to outside agencies such as homeless shelters, food stamps, medical providers, school‑based academic and emotional support programs, mental health services.
  • Providing services (in collaboration with the Supervisory Union Homeless Liaison) to homeless families and students, such as transportation, medical referrals, clothing, etc.
  • Involve community organizations and businesses in developing and implementing incentive programs for attendance, academic achievement and behavioral improvement.
Other Professional Responsibilities
  • Assists administration in establishing and maintaining school based discipline and a positive learning environment.
  • Any duties as requested by administration to support the school and students.
