Licensed Practical Nurse; LPN

Base pay range

$38.00/hr - $44.00/hr

Citadel At Casa Scalabrini is seeking a Licensed Practical Nurse (LPN) to join their team! The LPN assumes responsibility and accountability for a group of residents for a shift of duty.

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations
• Provides and receives nursing reports upon reporting in and ending shift duty hours
• Ensures assigned work area (e.g., nurse station, medication cart, medication room, etc.) and resident care rooms are maintained in a clean and sanitary manner
• Directs and supervises the day‑to‑day certified nursing assistant services for the assigned unit to assure care is rendered in accordance with current standards, guidelines and regulations
• Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care
• Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy as ordered within license limitations, and provides respiratory care (e.g., oxygen, nebulizer treatments)
• Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record ("MAR"), treatment/care plans as required
• Prepares and administers medications and performs treatments as ordered by the physician
• Charts nurse’s notes in an informative and descriptive manner reflecting the care provided and the resident’s response
• Completes required forms upon admission, transfer and/or discharge
• Ensures the resident’s environment is free from accidents and hazards, addressing potential safety risks promptly
• Makes written and oral reports/recommendations concerning shift activities as required
• Ensures all aspects of resident care plans are implemented and maintained; recommends care plan changes to the IDT per professional standards
• All other duties as assigned

• Excellent written and verbal communication skills in English
• Ability to multi‑task and remain calm under stress
• Analytical and problem‑solving skills
• Understanding and following infection control standards, policies and procedures
• Adaptability to a flexible work schedule and occasional overnight travel
• Basic computer knowledge and aptitude to learn company applications

• Current CPR certification required
• Current license to practice as a Licensed Practical Nurse in the state
• One year of experience preferred
• Long‑term care experience preferred

• Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, with or without mechanical devices required
• Limited potential for exposure to environmental hazards; understanding and adherence to company safety standards and protocols required
