Glen Haven Health and Rehabilitation, LLC is seeking a compassionate and detail‑oriented Registered Nurse to join our team as an Admission Nurse. In this role, you will be responsible for welcoming new residents, completing admission assessments, coordinating care plans, and ensuring a smooth transition into our facility.
Responsibilities- Complete admission assessments and required documentation
- Coordinate the admission process with residents, families, and healthcare providers
- Review physician orders and ensure appropriate implementation
- Communicate resident needs to the interdisciplinary care team
- Promote a positive admission experience for residents and families
- Current RN license in good standing
- Strong assessment and organizational skills
- Excellent communication and customer service abilities
- Long‑term care experience preferred
