Base Pay: $25.00-$29.55 per hour (based on experience)

Glen Haven Health & Rehabilitation is seeking an experienced and motivated Licensed Practical Nurse (LPN) Unit Coordinator to join our nursing leadership team.

As an LPN Unit Coordinator, you'll help oversee the daily clinical operations of an assigned nursing unit, coordinate resident care, support nursing staff, and work closely with nursing leadership and the interdisciplinary care team.
LPN Unit Coordinator Responsibilities- Coordinate daily nursing operations for an assigned resident care unit.
- Provide clinical leadership and support to nursing staff.
- Supervise, mentor, and support Certified Nursing Assistants (CNAs).
- Provide direct nursing care to residents as needed.
- Assess, monitor, and document resident conditions and promptly communicate changes.
- Administer medications, treatments, and nursing interventions according to physician orders.
- Coordinate resident care with Registered Nurses (RNs), physicians, therapists, and other interdisciplinary team members.
- Participate in resident assessments and individualized care planning.
- Ensure timely and accurate clinical documentation.
- Promote infection prevention, resident safety, and quality improvement initiatives.
- Support compliance with state and federal regulations and facility policies.
- Build positive relationships with residents, families, and fellow team members.
- Current Alabama Licensed Practical Nurse (LPN) license in good standing.
- Current CPR/BLS certification.
- Previous nursing experience required.
- Charge Nurse, Unit Coordinator, supervisory, or other nursing leadership experience preferred.
- Skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing experience preferred.
- Strong clinical assessment, communication, organization, and leadership skills.
- Ability to effectively lead and support a nursing team.
