Licensed Practical Nurse (LPN) – Charge Nurse | Evening Shift (3:00 PM–11:00 PM)

Full-Time | Hunter Creek Health & Rehabilitation | Northport, AL

Base Pay: $25.00-$29.55 per hour (based on experience)

Shift: 3:00 PM-11:00 PM

Hunter Creek Health & Rehabilitation is excited to welcome Licensed Practical Nurses (LPNs) to our growing team as we begin an exciting new chapter under the leadership of a new Administrator and Director of Nursing.

We're seeking a compassionate and dependable Licensed Practical Nurse (LPN) Charge Nurse to provide exceptional resident care while leading the nursing team during the 3:00 PM-11:00 PM evening shift

• Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
• Assess, monitor, and document changes in resident conditions.
• Administer medications, treatments, and nursing interventions according to physician orders.
• Lead and supervise Certified Nursing Assistants (CNAs) while supporting quality resident care.
• Coordinate resident care with Registered Nurses (RNs), physicians, therapists, and the interdisciplinary healthcare team.
• Respond promptly to changes in resident condition and communicate clinical concerns appropriately.
• Maintain accurate electronic nursing documentation in compliance with state and federal regulations.
• Promote infection prevention, resident safety, and quality improvement initiatives.
• Foster a positive, supportive, and team-oriented work environment.

• Current Licensed Practical Nurse (LPN) license in good standing.
• Current CPR/BLS certification.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
• Previous Charge Nurse or leadership experience is preferred but not required.
• Strong clinical assessment, communication, leadership, and organizational skills.
