DUTIES AND RESPONSIBILITIES

Under the direct supervision of the Manager of Youth Services, within the Youth Development Services-Diversion (YDS-D) program, the Diversion Case Manager will:

  • Provide individualized case management to clients participating in the YDS-D program, including developing a Care Plan and monitoring progress toward established plan goals.
  • Serve as the central point of contact for Youth through which a wide range of developmentally appropriate enrichment activities and services are coordinated for participating Youth, including the coordination of connections to activities and services available in-house and/or through additional local partnerships.
  • Duties include but is not limited to case management and program services for young people under the age of 18 in Geographical Area No. 16 – Norwalk, Cerritos, Lakewood, Artesia, Paramount and neighboring communities (include areas in SPA8), intake assessments, care plan development and revisions, client advocacy, and follow-up services, linkages, data entry, monthly reports and logs, annual reports, and other program related duties.
  • The case manager and the client will meet within five (5) business days of the referral to complete a comprehensive, culturally responsive, strengths-based intake assessment that will highlight the client's distinct service needs, i.e. mental health counseling, educational, employment, or financial services.
  • Be responsible for the coordination, planning, implementation, and execution of new Youth Development Services-Diversion program services.
  • Provide individualized Care Coordination guided by principles of Youth Development with a strengths-based, healing and trauma-informed, restorative approach to Youth.
  • Take a youth-centered approach and provide excellent customer service that is sensitive to the challenges that Youth and their families face, including but not limited to medical and behavioral health needs, as they maintain supportive services.
  • Tailor the level, intensity, and type of intervention to meet the individual strengths and needs of participating youth; reassessing goals to ensure relevant services are being delivered.
  • Provide meaningful experience and commitment to equity and community leadership in the relationship to the communities included in the program location and the inclusion of staff who are representative of the communities being served.
  • Maintain case file and provide accurate and timely documentation within agency documentation policies.
  • Demonstrate strong oral and written communications skills, including a sound foundation in English grammar, spelling, and punctuation.
  • Adhere to and promote HYCs mission and values. Acts as a role model to others exhibiting high degree of professionalism and ability to work with diverse populations and respect cultural diversity.
  • Capacity to be discreet and a demeanour to maintain confidentiality relating to client and personnel information.
  • Adaptive and multi-tasking ability. The individual must easily transition from one task to the next.
  • Ability to interact with employees of other departments in a professional and respectful manner.
  • Great work ethic demonstrated by reliability, punctuality, professionalism, diligence, and strong follow-up skills.
  • Must attend all necessary trainings and meetings.
  • Other related duties as assigned.
REQUIREMENTS
  • Bachelor's degree in a human services field or equivalent combination of education and relevant work experience.
  • Superb planning and organizational skills with a proven track record for strategic planning and execution and attention to detail.
  • Proficient in Microsoft program applications.
  • Ability to succeed independently within a fast-paced, dynamic, and sometimes unpredictable work environment.
  • Competency to work with the various ethnic/cultural groups in the target area/community.
  • Ability to work a flexible schedule, including evenings and weekends.
  • Great work ethic demonstrated by reliability, punctuality, professionalism, diligence, and strong follow-up skills.
  • Must be able/willing to travel as position requires cross collaboration partnerships, trainings, and meetings across LA County.
  • Must be able/willing to travel and work from all HYC offices.
  • U.S. citizenship or permanent resident status.
  • Valid Class C driver’s license. Current auto insurance coverage and good driving record.
  • Ability to successfully pass a TB screening test and Live Scan fingerprinting.
  • Ability to pass background check and criminal clearance.
