New Haven County, CT and surrounding areas

This role offers the autonomy of field work, the support of an experienced clinical staff, and the opportunity to build meaningful one‑on‑one patient relationships.

• Monday–Friday daytime hours for strong work–life balance

• Active Connecticut LPN license (required)
• Minimum 6 months of clinical experience (hospital, LTC, SNF, rehab, med‑surg, or similar)
• Valid driver’s license, reliable vehicle, and ability to travel to visit clients
• Strong assessment, communication, and documentation skills
• Spanish speaking is a plus!
• Home health experience is a plus!

• Provide skilled nursing visits in the patient’s home
• Document vitals and care during visits, and report changes in condition
• Implement and update individualized care plans under RN supervision
• Deliver patient and caregiver teaching
• Assist with medication management and wound care
• Communicate effectively with physicians, RNs, therapists, and the care team
• Maintain professional boundaries and promote patient safety at all times
