Primary Care - Nurse Practitioner and Physician Assistant

Position:
Nurse Practitioner (APRN) or Physician Assistant (PA) – Full‑time, 100% Outpatient Primary Care in Manchester, Connecticut.

• Schedule:
  
  Full‑time, Monday‑Friday (No weekends, No call required).

Provide routine physical examinations and comprehensive patient care in an adult medicine outpatient setting. Work collaboratively with physicians and care team members to develop and execute patient care plans.

• Perform routine physical examinations and develop medical, educational, and psychosocial plans.
• Consult with physicians and care team to plan further evaluations and referrals.
• Provide treatment within scope as outlined by protocols and refer patients outside of scope to physicians.

• Licensure:
  Unrestricted State of Connecticut medical license (or eligible).
• Training: APRN (Family, Adult/Geriatric Nurse Practitioner) or PA‑C.
• Experience:
  
  Minimum 2 years in primary care or relevant outpatient setting.
