Wellness Nurse (RN and LPN)

Bridges by EPOCH at Norwalk—an illustrious memory care assisted living community—is looking for dedicated Wellness Nurses (RN and LPN) to join our compassionate nursing team. This role includes providing high‑quality nursing care for residents in our “neighborhoods,” ensuring medical needs are met and enhancing residents’ quality of life.

• Per diem positions available: 7 am–3 pm, 3 pm–11 pm, and 11 pm–7 am on weekends.

• Deliver nursing care to residents, including medication administration, wound care, and health assessments.
• Collaborate with other healthcare and support staff to develop and implement individualized care plans.
• Maintain accurate and up‑to‑date patient records in accordance with regulatory requirements.
• Participate in interdisciplinary care meetings and continuous quality improvement initiatives.
• Serve as a mentor to new or less experienced staff, fostering a supportive learning environment.

• Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active nursing license.
• Comprehensive knowledge of nursing best practices and regulatory standards.
• Excellent communication, empathy, and teamwork skills.
