OverviewCenturion Forensic Health Services is proud to partner with the Connecticut Judicial Branch to provide community‑based forensic and mental health services to the Court Support Services Division Alternative in the Community (AIC) programs. We are currently seeking a full‑time Case Manager to join our team at the Norwalk Alternative in the Community office located in Norwalk, Connecticut.
ResponsibilitiesThe Case Manager is responsible for assessment, planning, and coordination of transitional services for offenders. This includes working with the offenders regarding their reintegration from a correctional facility to the community that requires clinical services or clinical placement upon release in order to promote continuity of care.
Qualifications- Bachelor’s degree from an accredited university or college; degree may be substituted with experience on a year‑for‑year basis
- Two (2) years’ experience administering assessments and performing case management functions
- Bilingual English/Spanish preferred
- Must be able to pass background investigation and obtain agency security clearance
- Monday‑Friday, Mondays, Wednesdays & Fridays 8:30am‑5:00pm
