The Connecticut Institute for Communities seeks a Social Services and ERSEA Professional to support families in accessing vital health, education, and housing services. In this role, you will coordinate ERSEA (Eligibility, Recruitment, Selection, Enrollment, and Attendance) activities, conduct family assessments, and connect clients to community resources that address medical, social, and economic needs. You will maintain accurate documentation, ensure regulatory compliance, and collaborate closely with interdisciplinary teams.

Responsibilities

  • Conduct eligibility, recruitment, selection, enrollment, and attendance (ERSEA) activities for families and children.
  • Perform intake and family needs assessments and develop appropriate service or referral plans.
  • Maintain accurate, timely documentation and reports to ensure regulatory and program compliance.
  • Coordinate with healthcare, education, and housing programs to connect clients with services.
  • Engage in outreach to underserved communities and build relationships with partner agencies.
  • Monitor participant progress and follow up to ensure services meet identified needs.
  • Educate families on available community resources and program requirements.
  • Support data tracking, quality improvement, and outcome measurement for social service programs.
  • Collaborate with interdisciplinary teams to address barriers to care and social determinants of health.
  • Participate in staff meetings, trainings, and community events to support program goals.
