General

Summary:

The Medical Social Worker determines clients’ needs and helps them get support services, such as counseling, meal
services, and financial assistance for medications and/or living needs.
Maintains performance improvement activities within the department.

Education, Licensure, Registration and

Experience:

LSW OR MSW.

Minimal two years experience in Social Work.

Licensed in state LSW or MSW.

Essential Functions:

Assures that social service/discharge planning activities are integrated and coordinated with the hospital/LTC system by effective communication at all levels.

Provides services to specific patient population, including neonate, pediatrics, adolescent, geriatric and general population.

Is knowledgeable of and utilizes appropriate community resources.

Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job-related hazards.

Maintains standards for licensing in the social work field.

Follows the hospital exposure control plans/blood borne and airborne pathogens.

Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department’s/unit’s ability to meet its goals and objectives.

Standards of Employment:

Attends all mandatory education programs and can describe his or her responsibilities related to general safety and regulatory compliance.

Appearance is professional, neat, clean, and appropriate for the work environment. Wears proper photoidentification badge while on duty.

Maintains confidentiality of all hospital and patient information at all times, as observed by peers and management.

Attends monthly staff meetings and participates in monthly rounding process with manager to assure effective and timely two-way communications.

Participates in and completes required department specific education and training programs and can describe his or her responsibilities related to department safety and specific job-related hazards.

Follows safety and security policies including exposure control plans/blood borne and airborne pathogens.
