RN Case Manager / Nursing Supervisor
OverviewTo support our growth, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Nurse Supervisor to join our agency and serve existing and returning clients throughout Huron and Lorain Counties, and surrounding areas in North East Ohio. This hybrid role includes case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, new hire orientations, trainings, in-services, and supervisory reviews.
Travel to multiple counties is required, with a consistent schedule to fit personal commitments. Part-time or PRN hours available, with potential to expand into full-time for the right candidate, and option for long-term employment.
- RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
- RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- RN documents/communicates/coorninates patient status/change in condition to the care team.
- RN promotes personal safety and a safe environment for clients and coworkers.
- Willing to travel surrounding counties.
- Assist with case management, supervision, and coordination of care, including wound care, lab draws, and OASIS assessments when applicable.
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse with an active RN License in Ohio.
- CPR and First Aid certifications required (or obtainable prior to hire).
- 1 year experience in case management and home health required; 2+ years in home health preferred.
- Supervisory experience; wound care, lab draws, and OASIS experience a plus.
- Willingness to travel throughout all stated counties in the service area.
- Strong background in nursing, patient care, and confidentiality; HIPAA familiarity.
- Current Driver's License and reliable transportation.
- Desirable traits: reliable, honest, professional, compassionate, detail-oriented, accountable, friendly, respectful, and positive.
- Service Area:
Norwalk, OH + 1-hour radius including Huron County and Lorain County. - Job Location Options:
Norwalk, OH in Huron County, with consideration for applicants residing in adjacent counties if travel requirements are fulfilled.
& Schedule
- Part-time or PRN hours; potential to expand to full-time (hourly or salary) for the right candidate.
- No on-call requirements / No overnight shifts required.
- Part-Time RNCMs receive hourly compensation (paid weekly, Fridays).
- PRN: hours and shifts vary; some weeks may have low or no hours.
- Weekend, overtime, respite & PRN services offered when available but not required.
- Flexible and customizable schedule; typical hours Monday–Friday (8am–5pm).
- HCN may expand role to full-time for travel up to 2-hour radius.
- Part-time Hourly Wage: $35 - $40/hour.
- Quarterly bonus opportunities for part-time and full-time RNCMs.
