JOB TITLE

RN Case Manager / Nursing Supervisor

Overview

This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. At least 1 year of case management experience and home health agency (or equivalent) experience are required. Must have experience with OASIS and NON-OASIS assessments.

1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.

Service Area
  • Norwalk, OH + 1-hour radius including Huron County and Lorain County
Travel/Workload Expectations
  • Part-Time:
    Typical travel distances require up to a 60-minute daily radius (or less).
  • Full-Time:
    Additional cases can be secured to achieve 32-40 hours/wk if willing to expand travel up to a 2-hour radius to cover additional surrounding areas and counties (not required).
Job Location Options
  • Norwalk, OH in Huron County is centrally located to serve all cases within this service area; qualified applicants residing in or adjacent to any defined counties will be considered if travel requirements are fulfilled.
Employment Type
  • Part-time or PRN Hours | Long-term employment
  • No On-Call requirements / No Overnight shifts required.
  • Part-Time RNCMs receive hourly compensation (paid weekly, on Fridays).
  • PRN Nurse: hours and shifts vary week to week, including some weeks with low or no hours.
  • HCN will expand this role into Full-Time (hourly or salary) for the right candidate willing to travel up to a 2-hour radius.
  • Weekend, Overtime, Respite & PRN services are offered when available but are not required.
Schedule Type
  • Flexible and customizable. Schedules are built to fit availability and preferences.
  • Typical hours are Monday – Friday (8am – 5pm).
Compensation
  • Part-time Hourly Wage: $35 - $40/hour
Job Summary

As an RN Case Manager / Nurse Supervisor, you will coordinate and provide patient care and ensure optimal outcomes. Core skills include managed care, patient assessment, client care and treatment plan documentation, and discharge planning. Your experience in critical care, infectious disease care, home and community care, and supervising will help lead a multidisciplinary team. You will ensure effective communication with physicians, agency staff, service administrators, other caregivers, clinicians, and the client and family.

Responsibilities / Requirements
  • Perform client assessments while directing, planning, and initiating actions in home care settings.
  • Manage/supervise a team of RN, LPN, HHA, and Personal Care Aides.
  • Document/communicate/coordinate patient status or changes in condition to the care team.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Willing to travel to surrounding counties.
  • Minimum of one year of nursing supervisory experience; home care experience preferred.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse with active RN license in Ohio.
  • CPR and First Aid Certifications required (or can be obtained prior to hire through agency or accredited third parties).
  • 1 year experience in case management and home health; 2+ years of home health experience highly preferred.
  • Experience in supervisory roles, wound care, lab draws, and OASIS assessments is a plus.
  • Willingness to travel throughout the stated counties in the service area.
  • Strong nursing, patient care, and confidentiality background; reliable attendance and dependability.
  • HIPAA familiarity and medical terminology knowledge.
  • Current Driver's License and reliable transportation.
