General Summary:

A Registered Professional, or Licensed Practical, Nurse is responsible for delivery of optimal resident care through collaboration with all disciplines and the nursing process of assessment, planning, implementation, and evaluation. Directs and guides resident and family teaching and activities of LPN and NA while maintaining standards of professional nursing promotes a safe, caring, home-like environment for the resident and families.
Essential Functions:
  • Applies knowledge of illness, injuries, and diseases and uses available resources in assessment process.
  • Responsible for assessing resident status, intervening to assist the resident to attain or maintain the highest practicable level of physical, mental and psychosocial well being; communicating pertinent information to other members of the interdisciplinary team.
  • Proceeds with formulating the resident's interdisciplinary plan of care.
  • Conducts implementation of Interdisciplinary Plan of Care.
  • Conducts evaluation of nursing aspects of POC.
  • Demonstrates knowledge of State of Ohio Nurse Practice Act, hospital, Norwalk Memorial Home, and nursing policies and procedures.
  • Acts as a resource person regarding resident care, specialty care, orientation, and teaching of staff.
  • Performs other duties as assigned.
