Home Health RN Case Manager & Recertification Nurse (RNCM)
Job SummaryAs an RN Recertification Nurse, you will coordinate, re-evaluate and confirm medical necessity of continued home health care services, providing patient care and ensuring optimal outcomes.
With core skills in managed care, patient assessments, compliance, documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
You will submit updated documentation within compliance of all state, federal, and agency guidelines, and ensure communication among physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
Responsibilities- Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
- Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- Document, communicate and coordinate patient status and changes in condition to the care team.
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties.
- Hold at least one year of nursing supervisory experience; home care experience preferred.
- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse with an active RN License in Ohio.
- Minimum of 1 year experience in a home health agency or equivalent setting.
- CPR and First Aid certifications required.
- Experience with OASIS and non‑OASIS assessments required.
- Strong background in nursing, patient care, and confidentiality.
- Exceptional on‑time and attendance record, and consistent availability aligned to chosen schedule type.
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Willingness and ability to perform required physical demands.
- Consent and ability to pass a State & Federal background check (including fingerprints).
- 18 years or older, active driver’s license or state , US work authorization.
- Reliable transportation to all scheduled visits within the travel radius (minimum 2 hours).
Road Warrior position covering entire Northwest Ohio region:
Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas. Willingness to travel throughout the region is required.
Weekday position (Monday–Friday) primarily daytime availability (8am–5pm); occasional evening availability. No weekend requirements.
Employment TypeFull‑time, long‑term employment. No on‑call or overnight travel required; weekend service optional.
CompensationFull‑time annual salary starts at $78,000–$83,200 per year, plus PTO, mileage reimbursement, quarterly bonus opportunities, health, dental, and vision coverage (individual or family), and other benefits.
