JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)
Position Overview: This is a part‑time or PRN "Road Warrior" leadership position requiring travel across Northwest Ohio. The role reports directly to the NW OH Regional Program Administrator and involves case management, admissions, recertifications, transitional care, clinical reviews, and related services, as well as occasional skilled nursing visits.
Schedule and Service Area:
- Road Warrior across Northwest Ohio:
Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding counties. Applicants must be willing to travel throughout the region;
Findlay/Fostoria/Tiffin is a central "home base", but applicants may live elsewhere. - Weekday (Monday – Friday) daytime availability (8am–5pm), occasional evening, no weekend required, no overnight travel.
Responsibilities:
- Perform client assessments, develop and initiate appropriate care plans.
- Manage and supervise a multidisciplinary team of RN, LPN, HHA, and Personal Care Aides.
- Document, communicate, and coordinate patient status changes with the care team.
- Conduct case management activities including admissions, recertifications, transitional care, and resumptions of care.
- Perform backup skilled nursing visits as needed.
- Ensure compliance with state, federal, and agency guidelines.
- Maintain clear communication with physicians, staff, service administrators, and families.
- Travel to client locations as required.
Qualifications:
- Graduate of an accredited School of Nursing.
- Active RN license in the State of Ohio.
- Minimum 1 year RN experience with at least 1 year in a home health agency or equivalent setting.
- Experience in case management, admissions, discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS, and non‑OASIS assessments.
- CPR and First Aid certifications (or ability to obtain before hire).
- Strong documentation and field staff leadership skills.
- Excellent communication, patient care, confidentiality, and HIPAA compliance.
- Physical ability to meet required demands and travel up to 2 hours.
- Valid Driver’s License / State , reliable transportation, US work authorization.
- Consistent attendance and availability aligned with chosen schedule.
- Desirable: nurse supervision, people management, staff education, and relationships with payors, AAA, DoDD, county board, VA, etc.
Compensation: Part‑time / PRN $36–$40 per hour (plus mileage reimbursement), paid weekly via direct deposit. OT and quarterly bonus opportunities available.
