Fisher-Titus proudly serves the greater Huron County area’s 70,000-plus residents by providing a full continuum of health and wellness care from heart and cancer care to outpatient services such as lab, imaging, and physical rehabilitation.

A Registered Professional, or Licensed Practical, Nurse is responsible for delivery of optimal resident care through collaboration with all disciplines and the nursing process of assessment, planning, implementation, and evaluation. Directly guides resident and family teaching and activities of LPN and NA while maintaining standards of professional nursing and promoting a safe, caring, home‑like environment for the resident and families.

• Hours of Work – Dayshift (Monday‑Friday) with every third week on call commitment

• Applies knowledge of illness, injuries, and diseases and uses available resources in assessment process.
• Assesses resident status, intervenes to assist the resident to attain or maintain the highest practicable level of physical, mental and psychosocial well‑being; communicates pertinent information to other interdisciplinary team members.
• Formulates the resident’s interdisciplinary plan of care.
• Implements the Interdisciplinary Plan of Care.
• Evaluates nursing aspects of the POC.
• Demonstrates knowledge of the State of Ohio Nurse Practice Act, hospital, Norwalk Memorial Home, and nursing policies and procedures.
• Acts as a resource person regarding resident care, specialty care, orientation, and teaching of staff.
• Performs other duties as assigned.
