Job Summary

A part‑time or PRN RN Case Manager / Recertification Nurse (“Road Warrior”) who travels throughout Northwest Ohio to provide case management, admissions, recertifications, transitional care, clinical reviews, and other services. The role reports to the Northwest Ohio Regional Program Administrator and supports leadership development through practice guidance, staff education, and quality improvement.

• Perform client assessments and develop care plans in home‑health settings.
• Document, communicate, and coordinate patient status changes to the care team.
• Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
• Provide skilled nursing visits, including medication sets, dressing changes, and support as needed.
• Ensure timely and compliant OASIS/non‑OASIS documentation and comply with state, federal, and agency guidelines.
• Collaborate with physicians, agency staff, and service administrators to optimize client outcomes.
• Maintain reliable transportation within the required travel radius (minimum two hours). No overnight travel or on‑call duties.

• Graduate of an accredited School of Nursing.
• Registered Nurse with an active Ohio license (minimum one year of RN experience).
• Minimum one year in a home‑health agency or equivalent setting.
• Experience with OASIS and non‑OASIS assessments.
• CPR and First Aid certifications required.
• Strong background in nursing, patient care, and confidentiality.
• Excellent attendance and punctuality record.
• Basic travel ability and reliable vehicle (capable of two hours travel).
• Background check clearance and valid driver’s license or state .
• Desirable: nurse supervision, people management, staff education, and relationship building with payors.

• Weekdays Monday‑Friday, 8 a.m.–5 p.m. with occasional evening availability.
• No weekend or overnight work required.
• Travel across the entire Northwest Ohio region (Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding counties).

• Hourly rate: $36 – $40 per hour plus mileage reimbursement.
• Overtime and quarterly bonus opportunities available for full‑time staff.
• Paid weekly via direct deposit.
