Home Health RN Case Manager & Recertification Nurse (RNCM)

This is a Part‑Time or PRN "Road Warrior" leadership position with our agency, requiring travel as needed to all clients throughout Northwest Ohio. The role reports directly to the NW OH Regional Program Administrator and supports career advancement to assistant or full program administration, clinical administration, and other leadership positions.

Work consists of case management, starts of care (SOC), admissions, recertifications, transitional care, resumptions of care (ROC), clinical reviews, new hire orientations, in‑service and supervisory reviews, and input to improve training, education, and care practices.

• Requires occasional skilled nursing visits and services such as med sets and dressing changes.
• At least 1 year of experience in case management, admissions, discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and Non‑OASIS assessments, documentation, and field staff leadership.
• At least 1 year of paid experience with a home health agency or in‑home care provider is required; 2–5 years is preferred.
• Desirable but not required: nurse supervision experience, people management, peer or staff education, and the ability to build relationships with client payors and case‑management entities in Northwest Ohio.

Road Warrior position covering the entire Northwest Ohio region, including Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas.

• Willingness to travel throughout the region is required.
• Findlay/Fostoria/Tiffin is a centralized “home base”; applicants may reside in or near any part of the service area.
• The position is weekday (Monday–Friday) with daytime availability (8 a.m.–5 p.m.), with occasional evening availability and no weekend shifts required.

Long‑term employment for Part‑time or PRN RNCM/Recertification Nurses. No on‑call or overnight travel. Weekend services are optional when available but not required.

As an RN Recertification Nurse, you will coordinate re‑evaluation and confirmation of medical necessity for continued home health care services, provide patient care, and ensure optimal outcomes. Core skills include managed care, patient assessment, compliance, documentation, discharge planning, and expert nursing care.

• Conduct client assessments and initiate appropriate actions in home care settings.
• Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
• Document, communicate, and coordinate patient status changes.
• Promote personal safety and a safe environment for clients and coworkers.
• Travel to surrounding counties.
• Maintain at least one year of nursing supervisory experience; home‑care experience preferred.

• Graduate of an accredited School of Nursing.
• Minimum 1 year of experience as a Registered Nurse with an active RN license in Ohio.
• Minimum 1 year of experience in a home health agency or equivalent setting.
• CPR and First Aid certifications required.
• Experience with OASIS and Non‑OASIS assessments required.
• Strong background in nursing, patient care, and confidentiality.
