Job Title

Home Health RN Case Manager & Recertification Nurse (RNCM)

Overview

Part‑time or PRN "Road Warrior" leadership position requiring travel throughout Northwest Ohio. Reports directly to the Northwest Ohio Regional Program Administrator.

Key Responsibilities
  • Case management, admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews and other assessments.
  • New hire orientations, in‑service and supervisory reviews.
  • Provide input to training, education, and care practices.
  • Back‑up Skilled Nursing visits and services including med sets, dressing changes, etc.
  • Perform client assessments, plan and initiate appropriate actions.
  • Document, communicate and coordinate patient status to the care team.
  • Supervise a team of RN, LPN, Home Health Aide (HHA) and Personal Care Aides.
  • Promote personal safety and safe environment for clients and coworkers.
  • Travel to surrounding counties within the Northwest Ohio region.
  • Ensure timely documentation of patient condition and care needs in compliance with all state, federal and agency guidelines.
  • Communicate with physicians, agency staff, service administrators, caregivers and client families.
  • Participate in training and compliance meetings.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum 1 year experience as a Registered Nurse with an active RN license in Ohio.
  • Minimum 1 year experience in a home health agency or equivalent setting.
  • Experience with OASIS and non‑OASIS assessments.
  • At least 1 year of experience in case management, admissions/discharges, wound care, PICC/PORT/IV care, transitional care, lab draws and excellent documentation.
  • CPR and First Aid certifications required.
  • Strong background in nursing, patient care and confidentiality.
  • Excellent on‑time and attendance record.
  • Physical ability to perform required duties.
  • Reliable transportation with travel radius of at least 2 hours.
  • State and federal background check (fingerprints) and US work authorization.
  • Valid driver’s license / State .
  • Desirable:
    Nurse supervision, people management, staff education, and relationship building with payors.
  • People management: peer/staff education and development.
  • Success or willingness to build relationships with client payors, case manager / entity relationships as listed.
Schedule & Service Area
  • Road Warrior across Northwest Ohio (Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk and surrounding areas).
  • Daytime availability Monday‑Friday 8 a.m. – 5 p.m., occasional evening availability.
  • No weekend service required; weekend services optional.
  • No overnight travel or call requirements.
Compensation & Benefits
  • Pay: $36‑$40 per hour (plus mileage reimbursement) based on minimum 2‑hour travel radius and required visits.
  • Paid weekly via direct deposit.
