Home Health RN Case Manager & Recertification Nurse (RNCM)
Home Care Network, Inc. (HCN) is a nurse‑owned, locally led home health agency committed to patient care and professional growth. We are seeking a highly skilled, experienced, self‑motivated RNCase Manager to serve clients across Northwest Ohio in a part‑time or PRN capacity.
Position OverviewThis is a part‑time or PRN "Road Warrior" leadership position requiring travel throughout Northwest Ohio. The role reports directly to the NW OH Regional Program Administrator and offers a defined career path for advancement within the organization.
Key Responsibilities- Case management, starts of care, admissions, recertifications, transitional care and resumptions of care, clinical reviews, and other assessments and services.
- Conduct new hire orientations, in‑service and supervisory reviews.
- Perform back‑up Skilled Nursing visits, including med sets and dressing changes.
- At least 1 year of experience in case management, admissions, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and non‑OASIS assessments, documentation, and field staff leadership.
- At least 1 year paid experience with a home health agency or equivalent (2‑5 years preferred).
- Desirable but not required: nurse supervision, people management, peer/staff education, and relationship building with client payors and case‑management entities.
Road Warrior position spanning the entire Northwest Ohio region:
Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas. Willingness to travel throughout the region is required. Weekday (Mon‑Fri) daytime availability (8 am–5 pm) with occasional evenings; no weekend service required.
Long‑term employment for part‑time or PRN RNCM/Recert Nurses. No on‑call, no overnight travel, and weekend services are optional.
Compensation$36–$40 per hour plus mileage reimbursement. Overtime and quarterly bonus opportunities available. Paid weekly (Fridays) via direct deposit.
Job SummaryCoordinate and confirm the medical necessity of continued home health care services, provide patient care, and ensure optimal outcomes. Manage documentation, team communication, and compliance with state, federal, and agency guidelines.
Responsibilities- Perform client assessments and initiate appropriate actions in home care situations.
- Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Document, communicate, and coordinate patient status changes with the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel surrounding counties as required.
- Have at least one year of nursing supervisory experience (home‑care experience preferred).
Regional Program Administrator, or Clinical Administrator/Director of Nursing.
Qualifications- Graduate of an accredited School of Nursing.
- At least one year experience as a Registered Nurse with an active RN license in Ohio.
- At least one year experience in a home health agency or equivalent setting.
- CPR and First Aid certifications required (or obtainable prior to hire).
- Experience with OASIS and non‑OASIS assessments.
- Strong background in nursing, patient care, and confidentiality.
- Excellent attendance record and consistent availability aligning to chosen schedule type.
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
- Familiarity with HIPAA regulations and medical terminology.
- Willingness and ability to perform required physical demands.
- Consent and ability to pass a state and federal background check (including fingerprints).
- Minimum age 18, active Driver's License or State , U.S. work authorization.
- Reliable transportation to all scheduled visits, within at least a two‑hour travel radius.
