This role provides clinically based, risk reducing care and coordination that facilitates the delivery of cost‑effective quality healthcare along the continuum. It directly interfaces with physicians, care managers, health care teams, patients and their caregivers to achieve desired patient outcomes.

Excellent communication skills, both written and verbal, are required. A commitment to coaching patients on health‑promoting behaviors, an ability to listen and assimilate complex clinical information, and the ability to work autonomously and in teams are essential. Participation in quality improvement efforts, research and evaluation processes related to the management of patient care is integral to the role.

Manges individualized, outcome‑oriented nursing care through the nursing process for pediatric and adult patients. Provides care, treatment and services through the successful coordination and completion of assessment, planning, care delivery, cross‑continuum coordination and outcome evaluation in accordance with system, departmental and ambulatory policies.

ESSENTIAL FUNCTIONS
  • Medication Administration
    • Demonstrates knowledge of and follows SSH policies for administering and documenting medications.
    • Completes medication reconciliation process per policy.
    • Demonstrates safe procedures for medication and vaccine administration.
  • Plan of Care / Documentation / Patient Family‑Centered Care / Patient Experience
    • Develops, discusses and communicates a prioritized plan of care for each patient.
    • Updates individualized plan and documents outcomes.
    • Initiates patient outreach, identifies and documents needs and makes appropriate interventions.
    • Continues assessment and identifies care needs within nursing practice.
    • Documents all patient care following policy, standards and disease processes.
    • Assesses patient response to interventions.
    • Works on behalf of patient/family; seeks help to represent them when needed.
    • Raises ethical questions; seeks resources for ethical decision making.
    • Communicates and collaborates with health team members.
    • Facilitates care across the continuum.
  • Safety / Quality – Foster a “Culture of Safety”
    • Verifies patient identification with two identifiers before any procedure.
    • Complies with CDC hand hygiene guidelines and universal precautions.
    • Ensures environment meets regulatory requirements.
    • Understands individual roles in ambulatory codes/emergency preparedness.
  • Professional Development
    • Obtains at least 5 contact hours per year in area of practice.
    • Practices within the legal boundaries of the MA Nurse Practice Act.
    • Accepts and responds to feedback and recommendations.
    • Develops professional relationships with colleagues from all disciplines.
    • Demonstrates critical thinking and effective communication.
    • Stays informed about policy changes.
  • Technology
    • Accesses Outlook, Epic, Health Stream and Workday as required.
    • Utilizes software applications per nursing and departmental standards.
