Participates in resident care conferences to assist in implementation of care plans.
- Performs assessments on all new residents to obtain history, establish specific goals and complete the appropriate sections of the Minimum Data Set (MDS) and also writes progress notes according to established schedules.
- Assists residents and families with adjustments after initial placement, encourages resident participation in facility activities, and co‑leads Family Council meetings for residents’ benefit.
- Identifies and documents changes in affect, behavior or personality and advises the appropriate personnel.
- Assists in preparing residents and families with implementation of the discharge planning process from the facility (when possible) and completes discharge home follow‑ups according to the established schedules.
- Intervenes in all resident conflict situations as necessary.
- Assumes duties and attends meetings as directed by the Administrator.
- Assists with the management of residents’ finances and with securing funding for skilled nursing care services when indicated.
- Participates in planning and providing staff educational in‑services on Residents Rights, substance abuse, and the emotional/social aspect of resident care as needed.
- Acts as a liaison between the facility and Norwichtown Rehab staff to foster communication and continuity of care of residents.
- Qualifies as a Social Worker under the specified requirements of the State of Connecticut Public Health Code with a W/MSW in Social Work
. - Ability to read, write, speak, and comprehend English; ability to follow oral and written instructions.
- Interest in the psycho‑social needs of the aged and the chronically ill with the ability to work with the elderly.
- Ability to work with supervisors, co‑workers and facility staff in performance of duties.
- Ability to observe proper safety and sanitary techniques.
- Ability to work hours as scheduled based on requirements of the position/assignment.
- Excellent assessment, communication and counseling skills.
- Experience in long‑term care setting.
- Knowledge of state and federal regulations governing the social service function within a nursing facility.
