Overview
Hartford Healthcare is seeking a compassionate Revisit Nurse to provide follow-up care to patients across our network in Connecticut. In this role, you will conduct revisit assessments in homes or facilities, monitor recovery progress, update care plans, administer medications, and reinforce patient and family education. You will collaborate closely with physicians, therapists, and care coordinators to ensure safe transitions, early issue detection, and reduced readmissions.

- Conduct revisit assessments in patients' homes or facilities
- Monitor recovery progress and update care plans accordingly
- Administer medications as prescribed and reinforce medication education
- Reinforce patient and family education to support self-management
- Collaborate with physicians, therapists, and care coordinators to ensure safe transitions and early issue detection
- Support efforts to reduce hospital readmissions through proactive follow-up and care coordination
